Make a Referral
Making a referral is simple.
Complete the online referral form using the link below. I'll be in touch to discuss the referral, answer any questions, and ensure the service is the right fit for the child or young person in mind.
Before submitting a referral, please ensure that the appropriate consent has been obtained from the child or young person's parent, guardian, or legal decision maker where required.

Once your referral is received, we’ll connect to discuss the child or young person, answer any questions, and make sure the service is the right fit.
The Process
I’ll spend time learning more about the child or young person, their experiences, strengths, relationships, and goals. This helps me tailor the support to meet their individual needs.
As the journey comes to a close, we’ll reflect on everything they’ve discovered and achieved. They’ll receive a personalised Life Story, More About Me or All About Me book that captures who they are, honours their story, and gives them something meaningful to keep, revisit, and continue building on throughout their life.

Who can make a referral
Referrals Are welcomed from:
Out-of-home-care providers
Foster and Kinship Carers
Schools and education professionals
Parents and Guardians
Together we’ll explore the child’s story through age-appropriate, engaging, and therapeutic activities. Depending on the service, this may include exploring identity, relationships, family history, important life experiences, emotions, strengths, culture, achievements, hopes for the future, and making sense of past experiences.

Unfolding Life Stories provides Therapeutic Life Story Work and identity-focused support across Sydney South West and surrounding areas.
Services can be delivered in a range of settings, depending on the young person’s needs and circumstances, including home, community, education and other agreed environments.
